About the Ordering Process

  • Can I get my Birth Certificate from you if I was born anywhere in Washington State?

Yes, you can get your birth certificate from Clallam County Health & Human Services if you were born anywhere in Washington State from 1923 to the Present.

  • How quickly can I receive my certified record from your office?

For applications received Monday through Thursday, certificates will be available for pick-up Fridays after 1:00 p.m. or can be mailed upon request.  For applications received on Fridays, the birth certificate will be ready the following Friday.  The fee for a certified birth certificate is $20.00 per copy.

For an additional fee of $10.00 (total $30.00), the certified birth certificate can be expedited and picked up the following day after 1:00 p.m.

  • What are your hours and how can I get there?

Office hours for customer service are:

    Monday through Friday from 9:00-11:30 and 1:00-4:00

Self-Service Center hours are:

    Monday-Friday during regular Courthouse building hours from 8:30-4:30

  • Is there a difference between a "Certified Copy" and an "Original Certificate"?

No, you may hear this document referred to as a "Certified Copy" or an "Original Certificate".
Birth certificates issued are certified copies printed on special certified paper with a printed and raised seal.

  • Can I get a credit card-sized certificate?

No, credit card sized copies of birth certificates are no longer available. Not all of the required information was provided on them so they became invalid documents by federal law in 1989.

  • How do I get an Apostille for my certificate?

An Apostille is a document that authenticates notaries' or state officials' signatures for foreign use, e.g. foreign-born adoptions, marriages overseas, etc. Apostilles are prepared by the Washington State Secretary of State Office in Olympia, WA at:

Birth Certificate Questions

  • I received my birth certificate but I see errors - or - I have recently changed my name. How can I make changes or edits on my birth record?

The Affidavit for Correction form is on the back of the birth certificate along with instructions.  The certificate will not be available until the Department of Health in Olympia processes the change affidavit.  If you bring the original certificate into our office (during regular office hours), we will exchange for the corrected original at no charge. 

  • I received my birth certificate but why isn't the time of my birth listed?

If you were born between the years of 1949 to 1967, no times were reported on births. You should check with the hospital where you were born to see if they have retained original records of your birth which may indicate the time.

  • How come the Father is not listed on the Birth Certificate?

In many cases it is because the parents were not married at the time of the child's birth.  Unless both sign and file a Paternity Affidavit at the time of the child's birth, the father will not automatically be listed on the birth certificate.

To add the father, a Paternity Affidavit signed by both parents, or a Court Order specifically stating to add the father to the birth certificate, can be filed with the Washington State Department of Health to amend the birth certificate.

Death Certificate Questions

  • Can you tell me if someone died in Clallam County?

There is an application that can be filled out to request a search for a death verification (IF the death occurred in Clallam County).  The fee is $8.00 to search a 10 year period.  The result will confirm that the person either did or didn't die IN CLALLAM COUNTY and a specific date.  To order a copy of the specific death certificate, you would be referred to the Washington State Department of Health http://www.doh.wa.gov/EHSPHL/CHS/cert.htm

  • How can I get access to a list of individuals who have died in Clallam County in the last ten years?

Sorry, but there is no master list of names of people who have died in Clallam County available to the public. Customers may purchase death certificates by following the necessary application process.
