Birth and death certificates are important records that mark vital events in our lives. That is why they are called vital records.

Getting a Certified Birth Certificate:

For people born in Washington State during or after 1924, certified copies of birth certificates can be obtained by submitting a completed application to the Department of Health & Human Services Port Angeles Vital Records Office in the basement of the County Courthouse, Room 010.  Office hours are Monday through Friday from 9:00-11:30 and 1:00-4:00.  You may also use the self-serve center located just outside the office Monday through Friday 8:30-4:30. 

For applications received Monday through Thursday, the birth certificate will be available for pickup Fridays after 1:00 or can be mailed upon request.  For applications received on Fridays, the birth certificate will be ready the following Friday.

Certificates cost $20.00 each made payable by cash, check or money order to the Department of Health & Human Services, when the order is placed (exact change required).  For an additional fee of $10.00 (total $30.00), the service can be expedited.

People born before 1924 must order their certified birth certificate from Olympia.  This can be done in one of three ways:

  • By picking up an application form from either the Forks or Port Angeles office. 
  • By calling the Washington State Center for Health Statistics at 360-236-4313.
  • By visiting the Washington State Center for Health Statistics website at
    http://www.doh.wa.gov/EHSPHL/CHS/Cert.htm.

People not born in Washington State can go to

to get information and/or apply from other states.

Getting a Certified Death Certificate:

When a death occurs, and a funeral home is involved, certified death records can be obtained through the funeral home.  Certified death certificates can also be obtained from the Department of Health & Human Services Port Angeles Vital Records Office Room 010 for up to 60 days after the person's death, if the death occurred in Clallam County.  Customer service days and hours are Monday through Friday, from 9:00-11:30 and 1:00-4:00. There is also a self-serve center located just outside the office for dropping off applications and payment Monday through Friday 8:30-4:30.  

For applications received Monday through Thursday, the death certificates will be available for pickup Fridays after 1:00 or can be mailed upon request.  For applications received on Fridays, the death certificate will be ready the following Friday.  It could take longer if our office has not yet received the original death certificate.

You can request certificates be mailed to you or you can come back to pick them up during regular office hours. You also have the option of mailing in your request with your payment and copies of the certificate will be sent to you.

If the death occurred outside of Clallam County or beyond 60 days, you must get the death certificate from Olympia.  You can do this in one of three ways:

  • By visiting the Washington State Center for Health Statistics website at
